ATF Form 4

A Firearms Transaction Record, or ATF Form 4, is a thirteen-page form prescribed by the Bureau of Alcohol, Tobacco, Firearms and Explosives application for tax paid transfer and registration of firearm used to request approval to transfer a National Firearms Act (NFA) firearm subject to transfer tax liability. Typically submitted for a transfer to an individual or legal entity such as a trust. There is a $36.35 transfer fee per handgun/long gun/receiver and a $85.05 transfer fee per Class III items (all NFA items).
